If it's a mac issue, the most usual cause is that full keyboard access isn't enabled. Solve this by going to System Preferences -> Keyboard & Mouse -> Keyboard Shortcuts -> select the "All Controls" radio button in the full keyboard access section at the bottom of the pane (not in the scrollable area). This drove me nuts in Firefox on a mac for ages (especially why trying to use phpMyAdmin which requires tabbing through a lot of radio buttons that Firefox would just ignore!)
