Another good alternative at a pinch for testing is the SA2GO screenreader which 
is also free http://satogo.com/
And if you just want very basic accessibility checking then the firefox addon 
'fangs' http://www.standards-schmandards.com/projects/fangs/ is useful.
NVDA is really good for working with though if you're a frontend developer 
because they have *excellent* support for WAI-ARIA -easily comparable, if not 
way better than, the two big alternatives (winow-eyes and jaws) plus it being 
free makes it much easier to actually utilise in testing.
