Is your program crashing?

Are you letting your computer go to sleep?

How often is this happening?

Can you give us a dir of that directory?

 

With a polite shutdown it removes the file.

 

Then there's the question of what happens you have multiple ones running?
Name collision?

That's exactly my point.  I am seeing it even though None is checked.  Not
every decode is saved.  It seems to be intermittent.
