What happened to the idea that the TX Level should always be left at maximum
(top). That was drummed into us at the same time that setting the sound card
channels (in and out) at 0db and the receive level ~30 with nothing but white
noise.

Hi Gary,
using digital attenuation of transmit audio is not critical for signal
quality until high levels of attenuation. On receive there is a
requirement to find the sweet spot between swamping quantization errors
and maintaining maximum headroom for possible strong signals. With Tx
there is vastly more scope for setting level. Neither are hyper-critical
but more attention is warranted for Rx level setting, mainly due to the
unknown factor of potential clipping from strong signals.
