In 1.5.6 some work was done on removing duplicated namespace definitions. Some
stuff was left undone though for fear of client code using the API to construct
e.g a wsse:Reference and then not defining the namespace explicitly in a parent
node. Given that the next release is 1.6, we don't have this constraint any
more.
