Angel, I looked into this, and Amy actually opened a bug report for this
a long time ago:

I documented a possible way to generate the TOC.xml information within
the bug. I already have some scripts written that convert XML Schema
documentation into HTML, so I could probably easily modify them to
handle the EXSD (after all they are just XML Schemas files, profiled).
