We do have a banking partner, but we do use a memo that details the history of collection efforts, which I pass to our CFO through our Chief Program Officer for approval. Copy also goes to our CEO. I can then refer to the memo, if needed, when I'm in court. I've attached a sample.

Please let me know if there is any currently using a "Write-off" form.  We do not have a banking partner and therefore close our own loans.  I am in need of a form for the supervisor to sign each time a loan is written off.  Thanks for any help.
