You didn't mention the version of xterm (and since XQuartz no longer
bundles xterm, that might be old), nor whether some other application
might cause interference (a user reported a problem which is due to
a mesa application).
"/opt/X11/bin/xterm -version" reports XTerm(367). I was unaware that
XQuartz no longer refreshed the utilities.
As far as other applications, I usually have a few xterms open and emacs
(local and remote). The vi behaviour seems to normal after setting
TERM=vt420 instead of xterm. Other X applications, such as xdvi, are
painfully slow on occasion.
