xsl:document is mentioned in the 1.1 working draft, which has since been
withdrawn. It is in the 2.0 working draft.
Use xalan:redirect instead of xsl:document if you are using Xalan.

As far as I can tell, <xsl:document> is not part of the standard XSLT
language. (See http://www.w3.org/TR/xslt for the Official Word). Maybe it
was someone's custom extension to the language, or something that didn't
survive into the final version, or something under development as part of a
future version....
If you can tell us what you expected it to do, we might be able to offer an
alternative solution...
