xsltc has problems escaping special characters

I changed the synopsis because it was too restrictive, having to do with 
Greek numbering. Other bugs have been marked as duplicates of this one.
A number of tests reflect problems with handling of special characters.
attribvaluetemplate08 outputs text from an xml file that has an 'o' 
with an acute accent mark. The encoding is iso-8859-1. The character 
should be escaped as &oacute; 

Tests mk028, mk029, and mk054 (the xml spec) fail because of the same
problem: umlauted characters in the output should be escaped as 
&ouml; and &uuml; 

output04 is related. It shows problems with outputting numbered character 
entitities for special chars, such as &#169;
