I figured that using an index would likely be the best solution, but didn't want to mess with the interfaces. An index-based fix should be fine, but I did find that my patch was incomplete. I'm attaching a new patch in case anybody cares. The flaw in my fix was found by someone using and older version of our product which used Xalan v1.2 compiled with Visual C++ v6.
