I _think_ we recognize the XHTML DTD, when it's specified via
<xsl:output/>, and switch into XHMTL mode. At one point someone started to
sketch an explicit XHTML output mode which would set up the same mode of
operation, but I don't think that was ever finished. It could be, if
someone has a bit of time to invest...
