The root node of def.xml, like any XML document, is un-named. The "document element" or top-most visible element in def.xml, is a child of that root node. Your passed params are not propagated by a built-in template. To verify my theory, say <xsl:apply-templates select="document('def.xml')//def" mode="xyz"/> instead of what you have, and tell us if that causes the passed params to work.
