Shane: I assume that when you say "general extension namespace", you mean the
one that would be used by all extension functions, extension instructions
(like redirect), and extension top-level elements (if we have any). I would
think that extension attributes (like indent-amount) would also go here. I
agree that extensions that could apply to standalone XPath or DTM should be
in the same namespace. PIs cannot be namespaced, so they are out of scope for
this bug. (PIs can be qualified via "notations" if anyone cares.)

Should the SQL extension functions be in with the others? I think so.
