The problem with the bogus xml declaration is fixed. There is a slight limitation with html type output. If the stylesheet doesn't explicitly set the output method to 'html' and the output properties set on the transformer object don't set the output method as html, you will get indented, regardless of the indent setting for the output properties set on the transformer.
