Has anyone noticed inconsistent entity reference conversions in output HTML?
For example:

A source doc text node contains:
XYZ(tm), ABC(tm) and DEF(tm)
After applying an XSLT stylesheet with Xalan, the resulting HTML contains:
XYZ™, ABC™ and DEF™
It appears that Xalan is doing the conversion inconsistently.  (™ is
equivalent to ™ but some browsers choke on the decimal represention.)
