I can't reproduce the problem. I don't know how you can get the result "xmlns:xmlns:cms" and I don't think you can specify "xmlns" in exclude-result-prefixes. The Xalan-J processor should complain about the prefix xmlns. By the way, the current XSLT processors don't support <xsl:output method="xhtml"> yet.
Provide your test case, we will try to figure out the problem.
