This patch fixes the following problems:
* A <defaultDatasource> element was _always_ generated, even when not required
* <enterpriseBean> elements had broken links to ids in the generated ejb-jar.xml (due
to a recent change to the ejb-jar_xml.xdt template).
* The jndiName attribute of the <ejbRefBindings> element was being prefixed with an
"ejb/"
