The XML Declaration must be the VERY FIRST THING in your document, except
possibly for a byte-order mark. Remove the leading whitespace.

(See http://www.w3.org/TR/REC-xml, look at  [1] and [22], note that neither
permits an S (whitespace) before the XMLDecl.)
