try this:

DocumentBuilderFactory dbFactory = DocumentBuilderFactory.newInstance();
DocumentBuilder db = dbFactory.newDocumentBuilder();
Document document = db.newDocument();
Element root = document.createElementNS("MyNameSapce", "System");
root.setPrefix("vst");
root.setAttribute ("xmlns:vst", "MyNameSpace");
...

document.appendChild (root);

Element error = document.createElementNS("MyNameSpace", "error");
error.setPrefix("vst");
root.appendChild(error);
