Thanks to everyone for their help. The problem was in my code. An exception was not being caught by my code and was being caught by SAX parser, which is where is showed up in the stack trace.

Unless I'm misreading the BNF grammar, <?WGXPPrightHandPageStart pagePrefix="Page 1-" ??> is a legal processing instruction. Unlike comments and the hyphen, there's no restriction on using the ? as the last character of a PIData.
