I have done this some years ago because I did not find any other information about the "origin" of the calculation for the page layout: if you use the latex companion (2nd edition) on page 200 you can read: "The circle is at 1 inch from the top and left of the page. Dashed lines represent (\hoffset + 1 inch) and (\voffset + 1 inch) from the top" And everywhere else (for instance p 194) dimensions 1 and 2 are described as : one inch + \hoffset, one inch + \voffset These \hoffset and \voffset are not reported (in latex companion) as specifically TeX commands! I wanted to get rid of these "one inch" to provide the publisher a "camera ready" file and the way I did was an easy way to give a centered text with the text width and height given by the publisher...
