There is an XSLT stylesheet parameter which allows to specify the URI of a custom cover page. See parameter "title-page" in http://www.xmlmind.com/ditac/_distrib/doc/manual/xsltParams.html#xsltParams However you need to create the XSL-FO for the cover page by hand. A sample cover page (http://www.xmlmind.com/ditac/_distrib/doc/manual/samples/titlepage.fo) is included in the documentation of XMLmind DITA Converter (which is integrated in XMLmind XSL Utilty).
