Thanks Hussein for the quick response.  Corrupt image makes sense I
tried inserting in Word it had same behavior as XMLMind.

I will file this one under GIGO

There are 2 problems with the files you sent us:

[1] In a fo:external-graphic like the one below:
---
<fo:external-graphic src="url('file://c:/defects/duke.jpg')"
width="90%" border="0px solid" role="html:img"/>
---

"file://c:/defects/duke.jpg" is an invalid "file:" URL. It should be
either:

"file:/c:/defects/duke.jpg"

or:

"file:///c:/defects/duke.jpg"

[2] Even after doing that, "duke.jpg" is still not rendered by XMLmind
XSL-FO Converter. However using RenderX XEP gives me exactly the same
result. Hence "duke.jpg" is ``corrupted''. After opening this file in
the Gimp, it appears that its resolution is 65535DPI. After fixing this
problem, everything works fine with both XFC and XEP.
