Sorry but XMLmind XSL-FO Converter does not support the background-image property. This limitation is documented in http://www.xmlmind.com/foconverter/conformance.html (search for "background-image").
Our second question is, how one can set the "Header from Top" and "Footer from Bottom" property within the xsl. With any setting we always ended up with MS-Words' default page-margins.
This is standard XSL-FO. See the fo:simple-page-master child elements and attributes found in attached sample XSL-FO files. Notice how the size and position of the headers and footers vastly differ in the RTF files generated out of sample1.fo and sample2.fo.
