I found one more minor issue: the fo attribute keep-with-next.within-column="always" is not working

When I changed keep-with-next.within-column="always" to
keep-with-next.within-page="always", this solved the problem you describe.
This looks like a bug. If this is actually the case[*], we'll try to fix
it in the next release (planned for this fall).
