The XSL-FO you have generated is needlessly complex. You generate up to
5 nested fo:blocks just to represent the title of a section. A single block
containing text and fo:inlines suffices in this case.

You should EITHER simplify what's above by merging redundant nested blocks.
(Please remember that for Word, the structure of a document is basically a flat
list of styled paragraphs!)
OR
At least, not optimal, you should specify xfc:outline-level on the most deeply
nested fo:block.
