Have append the .fo file in a zip format.

contain enough information to reproduce the problem here at XMLmind.

quickly (!!!) generates a 131Mb (!!!) RTF out of your 41Mb (!!!) .fo file.
However:
1) You didn't send us any of the graphics files referenced in the .fo file.
2) XFC reports many, many errors,

which are normal given the fact that all graphics files are missing,
except that these errors also mean that all the graphics file URLs found
in your .fo are incorrect:

and not:

src="res\meta.gif"

Important! fixing all these URL errors may solve your problem.
Please be kind enough to send us a *formal* bug report. This bug report
must include a *short*, *indented*, XSL-FO file + all referenced
graphics files.
