So I've managed to get XFree86 4.2 working with an ATI Radeon 8500 LE with a big desktop spread across two monitors.
Short answer, include everything in the config file, and leave the DVI output disconnected until after X starts.
I'm using an ATI Radeon 8500 LE 128MB aftermarket board on an Intel P4.
The OS is RedHat 8.0 with all the rpm updates until late March 2003.
Two identical monitors are connected, one to the vga out, the other to the DVI out via the DVI to VGA connector.
I installed the rpm from the ATI site fglrx-glc22-4.2.0-2.5.1.i586.rpm
I ended up recompiling the kernel module as detailed in the Driver Release Notes on the ATI site (the same place the rpm), but I don't know for sure if this needs to be done.
Run fglrxconfig to build the ATI recommended XF86Config-4 file ... which doesn't work properly.
I used the Big Desktop option, btw.
Go and edit the XF86Config-4 file in /etc/X11
In the "ATI device section" change HSync2 and VRefresh2 to not have any spaces between the values. As in:
Option "HSync2" "31.5-82.0"
Option "VRefresh2" "40-150"
In the subsection "Display" in the "Screen" section make sure there is only one mode.
Now, disconnect the monitor connected to the DVI output. Reboot the machine.
You can watch the machine boot on the monitor connected to the VGA output.
When the machine reaches the gdm login manger (or whatever), plug in the second monitor to the DVI output.
Voila! Big desktop across two monitors!
What's happening? I think that the ATI driver is probing both the monitors, but is misinterpreting the data from the DVI output monitor. In my XFree86.0.log I was getting information on both the monitors, but the one connected to the DVI out was showing up as failing every resolution test the driver made. Even 320x170 at 60Hz. Thus when X was running, the monitor connected to the DVI out seemed to the ATI driver that it couldn't handle any of the given resolutions, so it just turned off.
There is a switch in the ATI device section to disable Plug and Play (DCC) probing, but it seems like X and the ATI driver just ignore the switch. Frustrating.
When the second monitor isn't connected until after X starts, X and the ATI drivers are forced to use the values in the config file, and everything works out. Remember that you should make sure the values you give it work before overruling the DCC probe.
I personally have the second monitor now connected to a switch box, which I switch off when booting, and back on after X starts.
I've included two XFConfig-4 files. The first one is my ATI fglrx driver with a big desktop. The second one is a XFree86 radeon driver with identical displays to both monitors.
I've trimmed some of the information.
The important bits for the big_desktop config file are the HSync2 and VRefresh2 values, and the Modes.
The important bits for the cloned config file are the Xinerama, the duplicate devices and screens, and the server layout of the screens.

I am impressed and depressed. I assume that you were having the same "device not found" problem I am having with my ATI Radeon 9000 Pro. I tried making the changes you suggested with no result. I have not recompiled the kernel yet. And I haven't seen anything like a suggestion in the documentation that I should recompile the kernel. I am not seeing the agpgart module when I lsmod. Is that what they are talking about? Can you refer me to a reliable procedure to recompile the kernel? I have already run the two make scripts in the /lib/modules/fglrx directories. Does that ruin any chance of a good recompile of the kernel? If I went through all that, do you think I will have to go through the same weirdness with the monitor cable? It would not be worth it.
ATI should be ... chastised ... for posting those "drivers". The only thing they are good for is driving people crazy.
