Hello, I had the same problem: four similar files, three worked, the fourth didn't. So I copied and pasted the text of one of the working files in the non-working file, but it was still malformed! Then I created a new file, copied the contents of the non-working file in that file, gave it the original name, and it worked. Bizar... it had nothing to do with the contents.
So maybe just try to copy the contents to a new file? I find it very strange, but it worked for me... If it still isn't apparent what the problem is after that, post the minimum-sized document to the list so we can actually see it. It's hard to make any useful suggestions otherwise.
