OK, leave Your smtprelay.tab empty, configure Your accounts, tell them to use
SMTP authentication using the _full_ email as username.
I think Outlook has an option that says something like secure authentication (
using SSL ), do _not_ check that button.
In this way _only_ authenticated users can relay through Your server.
