The Server antivirus filter will only impact emails going TO the server... not trying to read them FROM the server TO the client.
My guess is that the client is running Norton/Symantec Antivirus. It has an invisible proxy that retrieves the emails for Outlook... and if the scan takes too long, it'll fail. The newer version of Norton/Symantec is better at it, but it still sometimes has problems with really big emails. Just go into the AV control panel for Norton/Symantec and disable the email scanner. Download the large email.
