As you explained, the mail route from exchange to final server is :

Exchange -> ASSP proxy -> clamSMTP -> XMail -> Internet

And the 'normal' bounce back route from Xmail is :

XMail -> Internet or Exchange with smtprelay in cmdalias


As show in your last exemple, more than one bounce is send for ONE message
so seems Exchange/ASSP/ClamSMTP don't send multiple times the original
message (if so xmail will generate different bounces for different xmail
IDs).
Does xmail 'smail' logs show xmail really sending multiple times the bounce
(same ID) ?
Can you give us the entire xmail bounces mails received at exchange account
with the bounces complete headers ?

Others questions on your setup :
- Do you use the exchange server IP or fqdn name in the cmdalias file ?
- Does the exchange fqdn resolve to multiple ip's ?
- Does the final IP(s) that xmail will use to connect to the exchange server
are really on the exchange server ? so does Xmail really directly connect to
the Exchange server ? no more filters, proxies, firewalls, ...

Did you never receive multiple times the same message with same ID on
exchange for 'normal' (no bounces) mails before ?
If you never see duplicate received messages at exchange side, its very
strange because a bounce is just a another 'mail' !
