Hello Erwin,

SpamAssassin can modify the Subject header (I'm not sure if email clients 
can recognise other headers), using the configuration option:
rewrite_header subject [SPAM]
If you want to modify it yourself, the Subject header (and X-... headers) 
look like this:
Subject: [xmail] x header

I'm not sure if XMail can filter on country, but you can make a filter that 
does a reverse DNS lookup and compares the domain extension to something..
