This question is not related to the tools you use, but to the style
sheets used by these tools.
* CSS style sheets are used to render DocBook documents on screen. These
CSS style sheets are the work of XMLmind.
For example, XXE_install_dir/addon/config/docbook/css/docbook1.imp
contains the following rule:

guimenu,
guisubmenu,
guimenuitem,
guibutton,
guilabel,
guiicon,
accel,
interface {
font-weight: bold;
}
---
XMLmind CSS style sheets have not been designed to make a document look
on screen like what it will look in print. That's why you have found a
difference in the rendering of guilabel, guimenuitem, etc.
* XSLT style sheets (see
XXE_install_dir/addon/config/docbook/xsl/fo/inline.xsl) are used to
convert DocBook documents to XSL-FOs (which are then converted to PDF by
the means of FOP or XEP). These XSLT style sheet are not the work of
XMLmind. Therefore we cannot answer questions about these XSLT style
sheets here, in this mailing list.
