Chuck,

Could you confirm that you are using version 2.6 patch 1 or higher? 
There was a post stating that this was a requirement.

If you do not have 2.6 patch 1 or higher, I believe you are supposed to 
use the following line:

<include location="docbook/docbook.xxe" system="true"/>
