Generally headers and footers are never added to an XML document.

The XML document, which is in principle pure data, is transformed to ``a 
printable form'' which has the concept of page, page header, page 
footer, etc.

The standard ``printable form'' is called XSL-FO. See

The transformation (an XSLT ``style sheet'' -- see 
http://lupo.pixware.fr/reference/w3c/REC-xslt.html) which converts an 
XML document to XSL-FO can be parametrized to generate sophisticated 
page headers and page footers.

I'm sorry if this is a bit complicated (hum, more than a bit, in fact) 
but the whole idea about XML is to separate content from presentation.
