It looks like double superscripts/subscripts do not work.  For example,

   <sub>1<sub>2<sub>3</sub></sub></sub>

should generate a subscript 1 with a subscript 2 with a subscript 3 
give the stylesheet
entry:

    sub { vertical-align: sub; }

Basically, baseline is calculated relative to the "parent" inline box.
