It is possible to implement a nice outline view using a specific CSS with embedded buttons and a docbook specific command (written in Java or in a scripting language -- do not try to do this with a macro command). Is it hard to do so? Well, everything needed to do so is documented but the learning curve is probably too steep for a power user. We are too busy to implement this in the short term but we've added this feature to the wish list. (It is very likely that it will be implemented because it is easy and fun to do it.)
