In summary:
[1] Add this entry for your DTD in config/catalog.xml:

<public publicId="-//INRIA//DTD specification 20020115//EN"
uri="interstices/dtd/document.dtd"/>

[2] Close all the documents loaded in XXE.
[3] Disable or clear cache using using Options|Options, Schema Tab.
[4] Restart XXE.
[5] Use New with your "DOSSIER with Section" template (i.e. not an old
document possibly using another publicId like "-//INRIA//DTD
specification 20011201//EN")
[6] Click inside first <p> and using Edit menu, choose Insert. The list
of elements should contain <a>.
Of course, if this works, the button/menu works too (I have tested that
too).
Find attached to this email a configuration that worked for me.
PS: Template "DOSSIER with Chapter" is structurally invalid (red icon)
and XXE will be very user-unfriendly in such case.
