Add this to docbook.xxe (preferably by inclusion at the *end* of
docbook.xxe, think that one day you'll upgrade your XXE):

This is described in the Power User's Guide.
Note that doing this just changes the behavior of Insert or Replace
dialog boxes. You'll have to also update the macro commands invoked by
toolbar buttons (or comment out the toolBar element in the config.).
And yes, we acknowledge that this feature is not yet very well
integrated in XXE...
