What do you mean by "changing the name in the DITAMap"? Changing its "title" attribute? Its "id" attribute? Using "File|Save As"? May be XXE crashed during "File|Save" or "File|Save As". If this is the case, it has probably something to do with your working environment. You need to run XXE in a console and send us the messages printed on it.
