[XXE] Followup: "xmllint says it's valid, xxe does not"

There were two parts to this problem:

(1) The DTD specified the wrong DocBook version (should be 4.5).

(2) The "/etc/xml/catalog" pointed to very out-of-date DTDs.  (XXE's
were current.)
