This is done on purpose. Some add-ons are tied to a specific version of
XMLmind XML Editor. Installing them in the app package allows to
automatically uninstall them when  XMLmind XML Editor is itself uninstalled.

For the above reason, it's not recommended to do that.

Note that Options|Preferences, "Install Add-ons" section, allows you to
automatically install all the add-ons in "Application Support" if you
prefer to do so. See

For me, it's a bug in Firefox because the generated XHTML file is fine.
What confirms this diagnostic is that Firefox will display sample.html
correctly after you rename this file to sample.xhtml.
