Such configuration has been requested several times by our users. See

However we have decided not to create it. We don't think XForms will
ever be widely used. Though less capable, we think that (X)HTML form
elements are more than sufficient for most needs. This will be even more
so with the advent of (X)HTML 5[*].

If you change your mind and decide to go for (X)HTML form elements,
suffice to add xhtml.jar to your add-on and import
"xxe-config:xhtml/css/xhtml_form.imp" in your CSS and you are done. More

The DocBook 5 schema includes HTML form elements. We'll probably add a
demo in next release which will showcase this feature.
