This is almost certainly caused by the documents you are opening.
* You have opened a document for which no inclusion scheme has been
declared (e.g. XHTML).
OR
* You have opened say a DocBook document, but XMLmind XML Editor does
not detect it as being a DocBook document (e.g. DocBook 4+ but no
<!DOCTYPE> or DocBook 5+ but no namespace), hence no inclusion scheme.
Hover your mouse above a Document tab, you'll see useful information
about it in the tooltip displayed there.
Yes, the option to enable the include tool is
on. And I don't see any include tool.
This is not related to the above problem and I have no idea of what
could cause this.
If the "Include tool" feature is selected in Options|Preferences,
General|Features section and you have restarted XMLmind XML Editor, then
you should see the Include tool on the right side of the main window.

However, if the Reference menu is disabled, then all the buttons of the
Include tool will be disabled too.
