I've tried to reproduce this problem the best I could:
* Linux 64-bit/Java 1.7.0_17 (latest release) 64-bit
* XXE v5.6 (next release).
* A 2.2Mb DocBook 4 document containing two dozen reasonably sized
images (the DocBook source of an actual book published by O'Reilly; not
that big: only printed 600 pages; monolithic, does not contain that many
images)
* Apache mod_dav server running on my Linux box (thus very fast network
connection).
As expected, everything worked fine for me.
I'm sorry but it is very, very, difficult for us to reproduce the
conditions you have described in your bug report:
I used the JRE 1.7.0_15 under Windows XP (32 bits), attached is the java
console output (though I don't think there is anything relevant in there). It
does not happen for the desktop application no matter how slow is the network
or how big is the file. Anyway there has to be specific conditions for it to
happen:
- slow network connection
- big xml file with lots of modules/images

PS: I would suggest to retry on your machine after temporarily turning
off the anti-virus software, proxy, firewall, etc.
