I'm sorry but with XXE v6, out of the box, I cannot reproduce the problem you describe using the exact sample file you sent us, and this, whatever the output format.

May be this problem is triggered by the XSLT stylesheet parameters you use to style <screen>, <programlisting>, etc, elements. I mean, XSLT stylesheet parameters like:

* linenumbering.extension -- http://docbook.sourceforge.net/release/xsl/current/doc/html/linenumbering.extension.html

* highlight.source -- http://docbook.sourceforge.net/release/xsl/current/doc/html/highlight.source.html

which cause Saxon *extensions* to be executed.

Anyway the problem occurs somewhere in Saxon 6.5.5, an old, generally extremely reliable, XSLT 1.0 processor, which is no longer maintained by his author Michael Kay (http://saxon.sourceforge.net/saxon6.5.5/).

Therefore, there is not much that we, XMLmind, can do to help you.

If you don't use a recent version of XXE, my first suggestion would be that you try to reproduce this bug with the Evaluation Edition of XXE v6. See http://www.xmlmind.com/xmleditor/download.shtml

If this fails, my second suggestion would be that you try to find the exact combination of XSLT stylesheet parameters which triggers this bug.
