This bug is fixed in XMLmind DITA Converter
(http://www.xmlmind.com/ditac/) v3.1.1 which will be included in XMLmind
XML Editor v7.4 (to be released this month).

Under the pressure of deadlines, I’ve simply edited the final .pdf in
Illustrator ;-} but it would be interesting to know how I’ve managed to
break things this time and whether there’s something simple I can do to
avoid this strange output in the future.
