http://cvsweb.xfree86.org/cvsweb/xc/include/extensions/syncstr.h dates it to the initial XFree86 import of that file in 1994. In the copies of the X11R5 & R6 trees I have around (yes, I do have to do archaeology often enough that I keep those around), there is no sync extension in R5, and that comment appears in the header in the R6.0 version. X doesn't do "temporary" well and needs more janitors to clean those out.
